Jacket Separation in Suppressor

I took my Tirant 45 to the range (MSAR) today for the second time since I got it. First time, no problems on three different guns/pistons.



So today, I was very excited to add a fourth to the list today with my 17L's new threaded barrel. Started with the G21 without the can and it shot just fine.



I then confirmed I had the right piston in, threaded it on the barrel. It was a bit tough to get it threaded correctly while pointed down range, but I had it on nice and tight. Fired a round - 5 inches low. What? Fired another - 5 inches low again. Uh oh. Stopped firing, took off the can. I look inside and see this (although this picture was taken when I got home):







I cursed, a lot, thinking I had a baffle strike and had ruined the can. I put the suppressor away and kept shooting. And brooding.



Went home and disassembled the suppressor. And all of this fell out of it (a big chunk with some nudging):







I inspected all of the baffles, especially the first (blast) baffle and can't find any signs of a strike. I can see some damage to the threads of the piston and little is left of the piston o-ring:







I cleaned it out and reassembled. Seems ok. The threads on that piston are definitely a bit messed up and I plan to get a replacement. Mounts fine to the other pistons.



Any idea what happened?
